The game is supposed to immediately end when a player cannot exchange his tile for a free one. My understanding is a free tile is one in the grid, but without a token on it. The spare tile in three player game is not a free tile (but can be exchanged).
Thus if all tiles in the grid are covered, the game should be over because the player on turn cannot exchange his tile in hand for a free tile.
From the rulebook, when the end conditions are explained:

Immediately, when a player at the end of his turn is unable to exchange his Civilization tile, because there are no more free tiles on the board, or because the free tiles left are identical to the one he already holds.
In my opinion, the spare tile is considered to be a free tiles. This end condition can be satisfied when the active player has in hand a tile identical to the spare one AND there is not free tiles left on the board grid. Possible but difficult condition, indeed.
If there is 4 different tiles (1 in each player hand and the one you can exchange) that doesnt have a War on it, the game will never finish...
Sorry, but I don't understand why it's necessary to have a War tile to end the game.
Anyway, the second end condition (end of the ziggurats) is still reachable.

Yes, I had asked the author about this issue during the development of the game and the current implementation is correct: the spare tile can be used for the exchange and it usually prevents the end of the game even when the board is full.
